Appointement as legal guardian

hello,
there is one child of 14 years old have not both parents, he is residing at his fathers home with mothers dad and now mothers dad want a legal guardianship.
for legal guardianship certificate where he wants to apply or can he make affidavit of declaration as legal guardian?
