Evidence affidavit

I need to submit Evidence Affidavit.
Can someone share the format of the same.
Also how to mention the list of documents submitted at the end of affidavit (details if should contain).
This will help in submitting my evidence affidavit in my Divorce case.

You can approach your own advocate who will be preparing the witness evidence chief affidavit, your advocate knows the procedures.
