I think Jayesh has some problem with typing.
Dear Jayesh, just read the message box once again before you 'submit'. This would resolve the issue.
As regards documents are concerned, unless the facts are clearly stated it would be difficult to suggest a model. By the way, have you referred to standard text books on the subject? these books contain variety of standard formats. just try them as well.
