There is no specific performa ! Write down on a paper that why you are not agree with state forum's view? That is appeal!

click on General Information. There you will find the requisitions for filing an appeal or revision before the Hon'ble Commission.

any petition which is not in the form prescribed by the NCDRC is not accepted by the commission.
