CONSULT SENIOR LAWYER.

For format you can approach the Bank for which You are supposed to submit the report. All the Banks have their formats / requirements through NEC and they would provide the certificate format.

For procedure you may contact some senior lawyer who has been submitting the reports for other Banks.

The formats differ from bank to bank, you may contact the bank itself for the ready made format. Please contact some local senior bank legal advisers/lawyers for the procedures to be followed.
