Name change

We purchased shares when our daughter was living with us in her name which was at that time R.Sumrata Murthy. Then her name was changed due to surname hassles to that of Sumrata Krishna Murthy. Now all her financial, educational, property are in this name.

Her shares are still in the previous name and dividend warrants are coming in that name.

Many dividend warrants are lost and not encashed due to this and demat also is not done.

What should we do now? Is a simple affidavit is enough? Is it to be notarized? Or a court order is necessary?

you are required to gazette notification.

I attached herewith procedure for name change.

Note: If you have already gazette then no need, you have to submit same.

i ADVISE, FIRST PREPARE affidavit of chg of name with effect from xx-yy-2011,
Then Publish in Govt Gazette, the change of name.
Then Post it by Regd Post to All concerned with Regn No. of the same , notify the same. & get the Record up-dated accordingly.

These are repeated query, already ANSWERED MANY TIMES. .

yes along with the gazette notification regard the name change, she can submit her affidavit while the next encumberance
