Icici bank trust pf withdrawal details

Hi, I am working in ICICI Bank. I like to withdraw my PF amount from the Trust account. But, I do not know how to claim it. Can anyone of you advise me to claim it. If any website link available, please share it.

Fill out a Transfer Claim Form – Form 13 (Revised) and send it to the PF A/C with Exempted Establishment, specifying the Trust Name and Address. Try in EPFO website.

1) Whether the PF amount relates to your and your Employer's contribution relating to your previous employment
2) Or the amount relates to the PF amount contributed from your salary while working in ICICI Bank.

According to current EPF withdrawal rules, if you are unemployed for one month, you can withdraw 75% of your EPF Corpus. Moreover, the balance of 25% can be withdrawn if you are unemployed for more than two months.
You must complete Form 13 and provide all necessary documentation to HR at your previous job. The previous employer will send you a set of two letters, one addressed to the new employer and the other to EPFO, together with a cheque that includes the amount of your PF.
