How to get first class affidavit

Plzz help i need to prepare an affidavit for name change and get it signed from 1st class magistrate, where can i do all this in faridabad- INDIA?

Get it prepared from some typist in the court premises on stamp paper of Rs. 10/-. Go to Tehsildar office, they would get photo of yours printed on affidavit and the executive Magistrate would sign the affidavit.
