Dear Experts,
This is Balasailendran from Rajapalayam. I need to know the procedure of change of name in Tamilnadu Govt Gazette.
my name in birth certificate (only) is Cylendra Prasath, but my name in all other documents like passport, School certificates, ration card, driving license is Balasailendran. Now I need to change my name in birth certificate so in Tamilnadu Gazette. Kindly let me know the procedure to do this.

very simple work, you directly go at gazette print office and get application form and file and add previous name and new name proof xerox and get notary seal and sign and you have pay some amount around 500/-. wait one week the name chaged notification will come to your home address.

You should consult local municipal office to know the procedure to change your name in birth records. An affidavit and application should serve the purpose.
