If it is a real problem being faced by your company, question arises, whether your company CAs or the Company Secretary are quite incompetent to handle your company affairs?

Your query, "what are the forms we have to file with MCA? And detailed procedure?", clearly suggests as if yours is not a real problem, but an academic exercise that you have to write answer for. Otherwise, the CAs of the company and the CS (if employed) are very well aware of the forms and procedure under company laws.
