Affidavit of President of society

Respected Expert,

I am going to register a chairteable society.I am President of Society.Three members belonged to same family.Will you pls intimate me then what is performa of affidavit.
1) That I am the President / Secretary of the Society named ________________________________________________________________.





(2) That the desirous persons of the Society are not related to each other by way of blood relation or otherwise.

If 3/4 members belong to a same family then how can you insert clause no. 2 in that affidavit?
