Chang in date of indian passport

(Querist) 20 February 2012 This query is : Resolved 
passport issued as per date of birth shown in School Leaving Certifiacate. Now wan to change it as per Birth certificate issued by municipality. Passport authority asked get order from court. May i have a model form for submittting application civil court to pass reuied order.?
