Appointment of guardian for mentally retired person

Hallo sirs
To appoint a Guardian for a "Mental Retardation"person a provision is made under Sec 14 of THE NATIONAL TRUST FOR THE WELFARE OF PERSONS WITH AUTISM, CEREBRAL PALSY, MENTAL RETARDATION AND MULTIPLE DISABILITIES ACT, 1999. The powers are vested with the Local Level Committees to make an appointment of a Guardian for a mentally retard person i.e. with disability. For that a Application to be made in Form A to Local Level Committee. Now the Q is i m facing practical difficulty that i do not know exactly who is acting as the Local Laval Committee in the area of bombay if the applicant residence is at Malad.The Act states A local level committee shall consist of-
(a) an officer of the civil service of the Union or of the State, not below the rank of
a District Magistrate or a District Commissioner of a district.
(b) a representative of a registered organisation; and __________ but sir i do not know exactly to whom should i apply on my client's behalf pls guide
