In the case of surrender of Service Tax certificate,Is any prescribed ST form given for surrender of Service Tax certificate?

If Yes,then which form to be filled for surrender of Service Tax certificate?

If no,then tell me that only Application To Superintendent of Service Tax,Central Excise is sufficient or not for surrender of Service Tax certificate.which are attached below.If any error is presented in given attachment so please guide me through correction in my attachment.

Pls reply as soon as possible.

dear for surrendering your ST registration certificate u have to apply before the concerned superintendent of your area and then he will give u a user name n password for on line surrender of your certificate n then only u will surrender your certificate on line.
