Advice Required for Name Change in Passport

For name change in passport, an affidavit needs to be furnished as mentioned in the below link.

Following are my queries:
1. This affidavit needs to be on non-judicial stamp paper. Could you please clarify the value (Rs. 10/20/30/40/50/100 etc) of the stamp paper required for this.
2. After the affidavit is notorised, what is the exact procedure for publication in newspaper/gazette. I live in Pune and Times Of India is one of the popular newspaper in Pune. Can I approach them for the publication of the name change or do I have to approach any government official/department for the publication of the name change notification in gazette. Similarly, Dainik Jagran/Hindustan are the prominent newspapers in Patna(which is my native place). Should I directly approach them or should I approach any government/judicial body for gazette publication.
If you could detail the procedure, it would be very helpful.

Ans. Rs.100/-

Ans. You can publish the public notice in 2 local prominent newspapers one in english and the other in the local language in both at your current and permanent residential addresses.(2 at Pune and 2 at Patna)
If you could detail the procedure, it would be very helpful.
Ans. Once the advertisement is published, you can take the paper clippings of the same and attach with your passport application. The procedure stated here is only for the purpose of procuring Passport and nothing more.

Thanks Vishal for the reply.
One more thing:
Is it strictly necessary that the affidavit for name change in passport should be exactly in the same format as mentioned on the passport website.
Can I add few more lines in the affidavit like mentioning my Roll Number of Class 10 and my Pan Card number. Reason being it will make more sense to the affidavit to relate the affidavit to me as my Roll Number for Class 10 and Pancard Number would never change.

Dear Abhishek,
You live in Pune and you give reference of papers published in Patan. What is the link between prominent papers of India and Patna?
It is not the case of prominent newspaper in a particular place, but should be popular through the most parts of country, which means the newspaper of national importance, not local importance, either of Pune or of Patna.
Anyway, you may act upon the advice of Mr. Vishal, which in itself is complete to the most extent.
