Claim for recovry in Maharashtra under CPA

I wish to file a recovery claim against a commercial co. op. society under CPA in Mumbai at State Commission level due to high amount above 20lacs.

Experts kindly help me with the format for such application. I shall be obliged.

There is no specified perofrma for that purpose. You should in first line address State Commission and then should right the names of the parties in detail and then should put your claim systematically.
