for changing surname in 10 and 12 th cbse marksheets

dear sir ,

i want to change my surname from 10th and 12 (cbse) marksheet . i m from ajmer region.

in marksheet , thakur is written as surname but originally we are chadhar .

how can i change my surname ?
what procedure have to i apply ?
and what documents are required to make the change ?

please sir describe it in detail.

You have to affirm an affidavit before the judicial magistrate to that effect and thereafter get a paper publication.
