Need format for agreement between a patient and hospital

Dear Experts,

Ours is a Hospital which provide health care service (both in and out patient) to credit patients too, which could be TPA / Companies / Govt Schmes for which the Patient is entitled to produce ID card and authorization letter from respective company within 24 hours failing which they need to continue as cash patient (initially we are thinking to take some advance on nonproducing the same) and return on producing the same. So, i need to draft a legal agreement for this.

Property as collateral

I wish to avail an educational loan exceeding 15 lakhs from private financial institution and has agreed to vouch my house as collateral.

The house is in grandma's name who is no more. No will was registered.

My father has two siblings and both have agreed to not claim the house and are ready to give NOC for that.

I wish to avoid the option of registering house in father's name for the time being.

The legal department has refused to accept property as collateral claiming bad title and sighting reason that a claim may arise in future.

If father's siblings agree to furnish an authentic NOC, why should there be an issue?

Please suggest a way out

Amendment of charitable trust deed and removable of trustees.

Dear Experts,

I am unsure of how to remove the trustees as there is no clause included in trust deed. What procedure I need to follow?

Also, can you please let me know how to amend the rights of managing trustee. The procedure for amending is not included and no clause is mentioned in trust deed.

I want to know, how to identify notarised agreement or document,
What are the actual procedure to make valid notarized agreement or document!!!

Succession certificate n survivors certificate

Succession certificate is necessary in case of bank deposits and other money and service benefits in case of employee.

Succession certificate will be issued by the Civil Courts Only

For immovable properties succession certiticat is not necessary.

Certificate issued by the Tahsildar ( revenue dept) is not succession certificate it is survivors certificate. On the basis of this certificate u can get enter your names to the immovable properties.

Is it the above information is right?


Actually we are three brothers and one sister and mother.



Our father died in the year 2009 and still we have not transferred the commercial plot +House in our name (immoval property in our father's name) so as per above information can we transfer the immovably property in our name with the help of survival certificate (issued by tehsildar) no need of succession certificate in case of immovable property

Please do correct me if I am wrong

My agricultural land extent is different in dasthavej and other documents.

Sir,

I have agricultural land acquired as a gift from my late father in west godavari, andhra pradesh. It is shown as 98 cents in my dasthavej and adangal. When I applied for demarcation, concerned official (surveyor) came and took the measurements and got 98 cents. According to the register book he carried it is 100 cents (1 Acre). So he told we have to take register book into consideration and said By mistake it was entered as 98 cents in dasthavej. But i got 98 cents in adangal from meeseva. How can it be. Survey is stopped due to politics played by neighbours. This May survey is going to be conducted again. What i have to do now. is my land 98 or 100 cents?

Buying online

I am to BUY a MUSICAL INSTRUMENT. this COSTS about 18,500/- if buoght ONLINE ; but I doubt whether this can give me LEGAL OWNERSHIP for the article ? The INVOICE sent along with the article is usually "COMPUTER GENERATED" and at the bottom there a remark saying "This is a computer generated Invoice and Signature is not required. Correction of minor name

Hi Sir,

I had named my baby boy in the birth certificate in Chandigarh.
But there is spelling mistake in the name. We want to change it but department is denying to changing the name of my minor son.
Even I have made the affidavit but they said we can't change due to the ACT.

Please suggest me how can I change the name. Please tell me about the procedure and legal documents used and also their wordings.
