please let me know the format of affidavit in evidence filed by the plaintiff in district court.

it is like the regular affidavit filed in other proceedings.

Please mind that this affidavit is to be used in the place of a deposition recorded in court, therefore it is to be made as close as possible to a deposition, indicating the documents to be marked as Exhibits, while narrating the facts cogently.
