Doctor residence cum clinic

The property is leased by the government for residential purpose alone to the landlord. the landlord gives the property on rent to a Doctor for residentail purpose. Now the question is- can the Doctor use the use upto 500 square meter of the space for his clinic? even though it is clearly given in the lease deed that the premises shall be used only for residential purpose. If yes then please tell me what's the reason behind it?
