Where to get marriage certificate from ?

Me and my wife got married in the year 1997. Recently during applying for passport , we realised that we have only copy of Application form of marriage certificate dated 1997. Since it was only a copy of Application form ( NOT a marriage certificate ) , it was not treated as valid document at passport office.Can you tell me ;
1) whether I can ask for marriage certificate based on the copy of application form i am having ?

2) if yes , where i need to go in Pune ?
