IF DEATH MEANS CLAIMANT'S DEATH, LEGAL HEIR CERTIFICATE, fir, POST MORTEM, ADDRESS PROOF, AGE PROOF AND LEGAL HEIRS. PHOTOS. INSURANCE COPY, rc BOOK COPY OF THE VEHILE HIT IF INJURY THEN WOUND CERTIFICATE, AND rc, ic AND PHOTO.
