Change in particulars of dob certificate

(Querist) 29 June 2015 This query is : Open 
My name is Ishant. But in Dob Certificate it is Prince. and my parent's surname also differ from my other certificate. and this is a deceased case of my parents. I want to apply for passport. What should I do?? Please suggest.
