If you want to convert, there are religious ceremonies to be performed. Of course there is no law which prescribes procedure for conversion from one religion to another. The subject is governed by the ceremonies given in the religious books like Bible, Quran, Vedas etc. There is also no rule that the name must be changed after conversion. However, if you retain original Christian name people will presume her to be a christian. Hence, people change it by publishing the change in Govt. Gazette for which one form is to be filled in supported by affidavit.

All religions is equal to this globe, however some people were forcible converted to other religions for unknown reason. After some time later they re-converted [converted back] to their original caste, there is no special in it, one who return back to their original caste, they should performa pooja in the respective temple and simply live himself in the religion who wish to faith. It's not safe to publish this in media, gazettee etc as experienced by me in the open world.
