Proof Affidavit

In a consumer court complaint, after the opposite party has filed their version, the judge has now asked for both of us to file a Proof Affidavit. Please guide me in what all could be included in the affidavit apart from the basic facts of the case like:
My name, occupation, address and that I am the complainant
The complaint against the opposite party
Evidence submitted by me during the complaint
Relief seeked by me.

Is there anything else that is to be added to the Proof Affidavit?

Thanks

reg: part payment made for goods delivered

I run a furniture showroom. One of my clients made part payment of the goods and got the entire items delivered on the condition that he will make the remaining balance payment at home. He gave a blank cheque with the wrong amount without the signature to our delivery guys. For last one week we are trying to contact him but he is avoiding calls. What action can we take against this client, civil or criminal?

Property tax

I am a doctor from pune.
I practise in a flat in building which is about 27 years old. the building is residential.
Recently i applied in the corporation to get my property taxed as per commercial rates.
The ratable value previously was something like 2500.
now they have calculated it as per present rates and it is a whopping 24000. that is almost 12times the old ratable value.
Can we go to court against this?
