You have to apply for change of name to your local revenue authorities. On their confirmation, the same will be published in the local gazette and that effects for new name (it may be for inclusion of surname)

YOUwill apply for change of surname with the GOVERNMENT GAZETTE in your District, and will also have to publish PUBLIC NOTICE in two local dailies in your area where you resides.
