Normally, the cheque is prepared in the name of the Assessing Authority of the jurisdictional circle. However, u can consult ur assessing circle for proper advice.

Ask from the office with whom cheque and challans are to be deposited.

If in Bank doing Govt. business Bank would ask cheque issued in its name or like ABC Bank A/c UP VAT Government account.
