Dear Sir,

I am working with super spacieality hospital as executive in medical records dept. I have lot of problem to name change who is admitted pateint besicaly for delivery case

1. patient admitted with maiden name but birth certificate want after marriage name and they also having insurance policy with maiden name then how I will chnage the name please tell me.

What is procedures to keeping maiden name ?

What is procedures to to adopting the new surname ?

you should go as per the documentary record provided to the hospital and not by choice of patient.

You cannot change the name. Once a name is got entered in the records of the hospital by the attendants of the patient or by the patient the same name is to be retained.
