Can anybody send me format of the sworn statement to be filed by the complainant in Private Complaint u/s 138 of NI Act???

No format for NI 138 cases , pleadings will be deffrent for every case.

Sworn statement should contain the name and address of the complainant, the name of the person who issued the cheque, the legal obligation for the cheque i.e. why it was issued, when it was presented, the banker where it was presented, when returned, name of the returning bank, reasons for the return, lawyer's notice under 138 NI Act, reply if any, and then the prayer for punishment for the return of the Cheque without honouring the same.
