The format for application is also available at the end of the document.
The format for application to the authorities is a standard format of Govt of Orissa and we have to type the format in Non-judicial stamp paper of Rs.10/- and court fee of standard value (Rs.25/- for complaints below the level of SP and Rs.50/- for the complaints above the officers Superintendent of Police and above)have to be attached therewith.
This is a very effective organisation, from my point of view.

respected Mr Ajay Bansal sir,
this being submitted for those who have no idea regarding the police complaints authority.
actually I had seen in this site, someone asking for the format to be filled up in filling a complain against police officers to the police complaint authority. I had tried many times to find out that query, but could not, hence posted this in the legal document section for all perusal.
