  1. Affidavit on Rs. 20/- Non Judicial Stamp Paper (Rs. 100/- if the applicant is resident of State of Maharashtra), duly verified by Special Executive Magistrate (SEM) or Special Executive Officer (SEO) or Notary Public.
  2. Indemnity Bond on Rs. 100/- Non Judicial Stamp Paper, duly attested can be submitted as alternate to the documents mentioned in point no. 3 .
