Form for police verification for enrollment to maharashtra state bar council

Police Verification is now mandatory for Advocate's enrollment to Bar council. Bar council. Is there a specific form available for this because police stations seem to be unaware and are asking to get one. I want to enroll at maharashtra and Goa Bar

Question: Is there a specific format for Police verification form to be submitted alongwith the Advocate Enrolment form to Maharashtra Bar ? pls help is anyone has a template.
