Summons for judgment

Can somebody please provide me with the draft sample of Summons for Judgment as mentioned in Order 37

Rule 3(4) as the form No.4-A in Appendix B is deleted as per the High Court Ammendment (Bombay).

Can any body please give me some suggestions in the above matter
