Succession Certificate for Shares & Property

Hello

i lost my father few months back

and obtained legal heirship certificateon my mothers name, but forgot to mentioned one property details in that, also there are few physical shares in my fathers name which i wish to transfer ito my mothers name 

Can i include shares and property in succession certificate

Could someone provide me a format of the succession certificate, where property and shares are included
