I am married lady and have a marriage certificate and pan card wtih my first name A and husband's last name B. So my name is now AB. Now i want to add my earlier last name C in between. So that it wud be ACB.
What is the process and the documents i need to submit for that?

You can give a reputed news paper ad. and apply to local revenue officer with paper ad. attachment.You will get a certificate in 15 or 20 days. Thereafter you will be called with new name.

do i need to go for changes in marriage certificate for that to the same place where i got my marriage certificate?

If you are belongs to Andhra Pradesh State, You have to submit an application along with paper ad. at your residential area Mandal Revenue Officer (Tahasildar).
