Seems like the insurance company is trying to pull  fast one on you giving whimsical reasons and just testing your patience. Action? Send a legal notice.

 

PS: Remove the Attachment ... its got wayy too much personal information.

So it will heard in Consumer forum OR normal courts .. please also suggest under which category it can be filed.

It's very much a consumer forum issue ... the insurance company will of course try to avoid consumer forum with the standard al cheapo tactics stating that it requires detailed evidence beyond summary procedure, but any decent lawyer would know how to answer that.
