Affidavit name change

I am an undergraduate student aged about 20 . I have recently changed my name after admission into college . However my class 10 (ICSE )and 12th (CBSE)boards have my old names . There is a procedure for name change in ICSE board but there isg no such procedure for name change in CBSE (as per new rule ) board examinations .Now the problem is that before applying for passport and student visa , i am at a fix whether to update my class 10th certificate to a new name with the class 12th certificate remaining in the old name leading to same names in class 10 and college degrees and my old name in class 12th certificate . Please advise what are the steps to complete before applying for passport (alongwith the possible altercations which may arise )

You can get an affidavit duly notarised stating that both the names are one and the same and if need be you may get it published in the government gazette publication which shall be an authentic evidence for the purpose.
