taxation errors by bank

My parents held an fd in a bank.after my dad demise I intimated the same to the bank to make necessary changes in fd. To my horror they informed me that 15H submitted by mother id not valid as my dads name still exist as first holder. Bank failed to inform me about their clerical error and my mother as widow paid the tax penalty for 3 years what is the procrdure to address my grievance legally.
