Reply to explanation for negligence in performance of duty

Dear Sir,

Please advise me for replying to action taken on veral advise of senior officer.  "Blank cheques have been to reporting officer for obtaining his signature so that official payments can be released proptly with the signautre of other officer.  As immediate officer was on long leave"  

 

Top management is now treating it as HIGH DEGREE OF LEGIGENCE IN PERFORMANCE OF DUTIES AND IRRESPONSIBLE BEHAVIOUR.

Without providing full background and details, members can not guide you.  It is apparent that as there is negligence in sending blank cheques )without filling the payee's name and amount,)  If there are laid down norms and even as a common practice  no cheque should be sent to higher authorities, who generally sign believing his subordinates and pressure of work.  (Superiors generally do not admit that signing cheque without verification is negligence and they justify belief and work pressures).  Do not repeat violation even in your own interest.
