How can i renew my ration card mumbai

We changed our Home 5 Years Back

Now from somewhere we came to know that our Ration Card has been cancled

Please tell me how can i know weather my Ration Card has been cancled or not

and if cancled that how can we renew it and also change the address to new one

Any Ration Card Agent in Mumbai ??

If you have old ration card, you visit to your collectorate office, there is a supply dept. working on this issuing of ration cards. They will give a application form , you attach a old one and furnish the form along with new residential address proof. Subsequently you will be issued with Ration Card after paying some stationery charges etc.

I have visited the the Nearby Ration Card Office and they said that my ration card has been cancled and it cannot be re-activated

so The only thing i can do is to apply for a New Ration Card

The Problem is my Old ration card was created (issued) in 1989 and Rationing Officers say the new card will be issued from current year 2013 and i will lost all old data (proof)

Can you tell which Form (form no. or name) Do I to submit to re-active my Card

or any Agent Contact Detail who can help me out in this
