Forwarding application through proper channel

Dear Sir

         I am working in geological survey of India as geologist for the past one half years. I would like apply to other govt organisation through proper channel. I came to konw that I must give an undertaking before routing my application throuh proper channel. From where I can get the format for this undertaking? If you have one kindly send me. Please help me in this regard
