Company was not accepting my spot letter

Greetings,

              Currently im working in a MNC Company(BPO).I got some health issue so i cant able continue on work.So i asked for spot relieving But they saying it is impossible is there any option to deal with it,I am working there for past 15 months.Kindly provide me suggestion ASAP
