Child surname change

Dear Sir / Madam     

I am the legal guardian for my daughter. I want to change her surname as i am remarried. she is 16yrs. 

Is it possible to use  a sworn affidavit or Annexure E for college and school purpose without changing in the passport.

If i do a notary public with the help of my advocate will that  document be sufficient.

WIth out the consent of your former husband you cant change the name of your child . Once the child attains majority that is once she has crossed 18 yrs of age the child can change her name .

I have complete custody of the child.  i dont want to  change anything in the  passport,  birth cirtificate, just   can she be from now on wards be known as so and so for the school and college purpose
