I've applied for passport as PRASHANT KUMAR TRIPATHI, having PRASHANT KUMAR as given name and TRIPATHI as surname. I'm having documents in which name is mentioned as PRASHANT KUMAR.
What can I do to add my ssurname in passport??
Do I need to apply for name change???

Yes, apply for name change in newspaper and an affidavit to this effect. Take the newspaper and affidavit with you while your appointment in Passport office.
