Plz guide me regarding surname change

From where to get affidavit for surname change? Is it a stamp paper? On what conditions does notary attests affidavit? Who can be witness? Further procedures etc.?

Surname cannot be change. Do not waste affidavit. Declaraton suit has to be filed for change of surname in the civil courts. Better consult a civil lawyer with all documents at your area.

But there is notification from dept., which says that affidavit is to be get attested with notary.
