Non release from delegated service by State Govt

There is a delegated service known as NRC for the updation of citizenship registration in Assam. It has many centres in and around districts of Assam. It has been assigned to many Govt officials officiating the parent duty as well as delegated duty of NRC. Kindly give some solution to a person suffering from heart problem but he doesn't want to give full descripttion of his disease otherwise he may be demoted from his parent service
