Noc format for name change in property tax bill

I want to change the name in Property Tax bill, I received. I have been asked to submit the NOC from Society along with the relevant document. Can any one provide me the NOC fomat for Name Change in Property Tax bill? So that I can submit the same with appropirate information filled to the society and get it typed & signed on Society letter head.

No Objection Certificate
To,
The Accessor and Collector of Tax,
PMC Pune.

This is to certify that Mr. Xavier, resident of Flat no. 403 is a member of our society naming xyc Co-operative Housing Society Ltd. Warje, Pune 52.
Mr. Xavier intends to sell their above mentioned flat to Ms. Elizabeth.
The society has no objection to sell subject Flat 403 to Ms. Elizabeth.
We confirm that society has not issued Share Certificate yet. Also there are no outstanding dues payable by the member in respect of said flat and he paid all the taxes/dues in respect of the same up to date. We further confirm that he transfer his property in name of Ms. Elizabeth as per rules and regulations of the society, we have no objection for that.
