Visit the PRO/APFC and if required RPFC nearest PF office. If you have the PF number you can have all info from EPFO.
The BM of your bank is one of the designated authority to attest the form. You may submit the attested PF form along with proof of having submitted the PF forms to company and company not attesting it, thru RPFC in nearest PF office under acknowledgment.
