Name change i gazette

Hi Experts,

My mother's name befor marriage was Pushpam and after marriage she changed her name as Latha. Later on, in all the documents her name was Latha including the ration card. But in my birth certificate, Its mentioned as Pushpam. This stands as a obstacle to obtain an Passport for me. When I approched the passpost office, they are asking for a name change in gazette. How can I do that? For doing this do i need any document which has my mother's old name?(Pushpam).Kindly help me in this. Thanks in advance...

There  is a format of affidavit for name change for specially for women. Women generally names are different before and after marriage. such people can submit an affidavit with notary. Search the same in passport website. You can find it the proforma.
