Format for applying for Certified Copies of judgement?

Is there a specific format for applying for Certified Copies of the judgement of Metropolitan Magistrate of Mumbai or one can give self written application to Court Registrar ?

There are different forms for certified copy of civil and criminal cases as prescribed by Bombay High Court.
